Case Study: Ingrebourne Medical Centre

How a Small GP Practice Built an Outstanding Quality System

Executive Summary
Ingrebourne Medical Centre demonstrates that outstanding governance is not determined by the size of a practice or the number of managers. With the right Quality Management System, a small, well-organised team can outperform organisations with significantly larger management resources.

Using the everythingCQC Full Quality System, the practice has implemented a comprehensive, ISO 9001:2015-based quality framework that provided every process, policy, protocol and evidence log needed to be inspection-ready at all times.

Combined with the commitment of the management team, this created a level of organisation that changed the CQC inspector's expectations.

Client Experience

“I just wanted you to know that the CQC Inspection went better than we could have hoped. We are getting a Good rating. He said he was very impressed and he’s even getting his parents to register at our practice.
We thank you for help and we look forward to continuing with your great work!”

The Challenge
Small GP practices are often assumed to have limited governance capacity.
At the start of the inspection, the inspector shared his expectation that:
"Small practices are usually the weakest. They have good GPs, but their governance is often non-existent."

Like many single-site practices, Ingrebourne Medical Centre had limited management resources. Building and maintaining hundreds of compliance documents, policies, risk assessments and evidence folders manually absorbs enormous management resources.

The Solution
Rather than investing valuable in-house resources, the practice implemented the everythingCQC Full Quality System. The system provides a complete, ready-made Quality Management Framework including:

  • An Operations Manual with structured protocols
  • Policies aligned to current legislation and best practice
  • Risk Assessments
  • Evidence Logs
  • Governance processes
  • Automatic planners and review schedules

Built around ISO 9001:2015 quality management principles, the system required very little implementation time and virtually no ongoing maintenance, with updates managed automatically.
This gave the practice the type of governance infrastructure normally associated with much larger organisations.

The Inspection
The impact was immediate.
After reviewing the Quality System, the inspector's opinion changed completely.

He commented:
"If you complete the system, I'll have no problem giving you an OUTSTANDING rating."

Following the inspection and interview with the GP Partner, his final comment was even more telling:
"I'm going to register my parents with your Practice."

For the practice team, this was perhaps the strongest endorsement possible—not simply confidence in compliance, but confidence in the quality of care being delivered.

Why It Worked
Although the Quality System provided the framework, success ultimately came from the people using it.

Practice Manager Angelo Soteriou and Assistant Manager Leanne Dooner used the system to create an organised, structured approach to governance where everyone knew:

  • where the practice stood
  • what needed to be done
  • what should be prioritised
  • how to prepare for inspection
  • how to maintain compliance continuously not just reacting to inspections

Instead of spending countless hours on policies and building spreadsheets, they were able to focus on improving the practice.

As they explained:

  • From day one, we knew exactly where we stood.
  • We had a clear vision of what needed to be done.
  • We had a roadmap for becoming inspection-ready.
  • We no longer had to create our own compliance system from hundreds of separate policy documents.
  • We knew we were investing in a future-proof system that would support the practice for years.

A System That Prioritises What Matters
One of the greatest strengths of the Quality System is that it removes uncertainty.

Rather than presenting managers with hundreds of disconnected tasks, it produces clear, objective and risk-prioritised action plans.
Automatic planners and review schedules ensure that high-risk activities are addressed first, allowing managers to remain in control even when workloads are high.

The result is confidence. Managers always know:

  • what needs attention
  • what can wait
  • what evidence is required
  • what has already been completed

The Outcome
Ingrebourne Medical Centre demonstrated that outstanding governance is not about employing more managers.
It is about giving capable people the right tools.

A small management team, supported by a structured Quality Management System, was able to achieve a level of governance that exceeded the inspector's expectations and rivalled organisations with far greater administrative resources.
While everythingCQC provided the framework, the inspection outcome reflects the professionalism, organisation and commitment of the practice team. Even the best system cannot deliver results without dedicated people using it well.

Together, the team and the Quality System created a practice that was organised, confident and inspection-ready at all times, a key element to allow GP Partners and the clinical team to focus on care and perform at their best.
